Sedona Staffing is seeking a temporary
Reintegration Advocate
for a local
non-for-profit organization in Rock Island, IL
. This position would be responsible for the transitioning of
persons
of all disabilities, ages 18 to 59 (any age; HIV/AIDS or Brain Injury Waiver) out of nursing homes through an Illinois grant which includes screening and intensive follow up to make sure successful of a transition into the community.
Responsibilities:
- Conduct outreach to nursing homes and community partners to identify individuals interested in transitioning from institutional settings to community-based living.
- Conduct community intake interviews and assessments to identify consumer strengths, goals, eligibility, and service needs.
- Develop individualized aftercare plans and assess consumers’ current and future needs to support successful community reintegration.
- Attend consumer transfer and transition meetings with local Illinois Department of Human Services (DHS) staff and other service providers as appropriate.
- Attend statewide Community Reintegration Program (CRP) quarterly meetings and complete all required trainings .
- Maintain accurate, timely, and confidential consumer records in accordance with program requirements using State of Illinois web-based systems.
- Complete required monthly reports and maintain accurate program data in the organization’s internal database.
- Assist the Direct Service Team with program changes, service-related issues, and other operational needs as assigned.
- Conduct outreach and develop community partnerships throughout the six-county service area in Illinois and Iowa.
- Provide consumers participating in the reintegration grant program with information and referrals, peer counseling, individual advocacy, and independent living skills training based on their identified needs and goals.
- Attend and participate in community meetings and disability-related workshops and educational activities.
- Provide support to other grant-funded programs and initiatives as assigned.
- Maintain effective working relationships with internal teams, community partners, state and local agencies, and other stakeholders. Participate in committees and collaborative activities as requested.
- Attend and actively participate in Direct Service Team meetings and staff meetings.
Requirements:
- Bachelor’s degree in social services, human services, or a related field preferred; equivalent experience in independent living or community-based services may be considered.
- Personal or professional experience working with individuals with disabilities and/or supporting independent living is preferred.
- Knowledge of assistive devices, supports, and resources that promote safe and successful independent living.
- Familiarity with Certified Nursing Assistant (CNA) and/or Personal Assistant (PA) services and their role in supporting individuals in the community.
- Excellent verbal and written communication skills, with strong interpersonal and relationship-building abilities.
- Proficiency with basic computer applications, including Microsoft Word, Outlook/email, and web-based software systems.
- Strong organizational, time-management, multitasking, and problem-solving skills, with the ability to prioritize responsibilities and meet deadlines.
- Self-motivated and able to work independently while also functioning effectively as part of a team.
- Professional, goal-oriented, and committed to improving the quality of life and supporting successful community transitions for consumers.
- Willingness and ability to work a flexible schedule based on program and consumer needs.
- Ability and willingness to travel throughout the designated service area.
